provider–patient relationship or guarantee access to any specific service, treatment,
prescription, or product.

and understand how my personal information may be collected, used, stored, and shared.

I understand that certain information I provide may be considered protected health
information (PHI) and may be shared with licensed clinicians, pharmacies, laboratories,
and operational partners solely for the purpose of evaluating eligibility for services,
coordinating care, and fulfilling orders, in accordance with applicable privacy and data
protection laws.

I understand that this platform may facilitate access to telehealth services provided by
independent, licensed medical professionals.

• Telehealth services may be delivered asynchronously.
• Medical services, prescriptions, or treatment recommendations are not guaranteed.
• A licensed clinician may determine that I am not an appropriate candidate for care.
• Creating an account does not initiate medical treatment or establish a provider–patient relationship.

I consent to receive electronic communications from the platform, including email,
SMS/text messages, and in-platform notifications, related to account activity, service
coordination, appointment reminders, order updates, and required notices.

I understand that message and data rates may apply and that I may opt out of
non-essential communications at any time, except for legally or operationally required messages.

I attest that:

• I am the individual creating this account and will not allow unauthorized access.
• All information provided is accurate and complete to the best of my knowledge.
• Providing false or incomplete information may result in denial of services or account termination.
